Data Deletion Instructions – EPGI Referral Management System

 

East Point Hospital is committed to protecting the privacy of individuals whose information is processed through the EPGI WhatsApp Referral Management System.

The system is used to manage hospital referrals and may process information such as patient names, age, WhatsApp phone numbers, referring doctor details, referral information, remarks, communication records, and related referral status information.

How to request deletion of personal data

Individuals who wish to request deletion of their personal information associated with the EPGI Referral Management System may contact the hospital using the official contact details below.

Please include the following information in your request, where available:

• Your name and contact details.
• The patient’s name or referral reference number, if known.
• A brief description of the information you want reviewed or deleted.
• Your relationship to the patient, where applicable.

For patient privacy and security, the hospital may request additional information to verify the identity and authority of the person making the request. Please do not send passwords, access tokens, or unnecessary sensitive medical documents through ordinary email or WhatsApp.

How the hospital handles deletion requests

East Point Hospital will review each request and determine the appropriate action in accordance with applicable laws, hospital policies, medical-record retention requirements, and legitimate operational requirements.

Where deletion is permitted and appropriate, the hospital will arrange for the relevant personal information to be deleted or de-identified, as applicable. Where information must be retained for legal, regulatory, medical-record, audit, or other legitimate requirements, the hospital will explain the applicable limitation where appropriate.

Requests will be handled securely and within the applicable legal or regulatory timelines.

WhatsApp communications

Individuals may also stop further WhatsApp communications by informing the hospital. Stopping WhatsApp communications does not automatically delete referral information or hospital medical records. A separate deletion request may be required.
